Vasco improved as the match progressed and took control in the second half after a cautious opening against a well-organized Medellín defense.
The introductions of Brenner and Adson gave the Brazilian side more attacking threat alongside right-back Paulo Henrique, under the guidance of new head coach Pedro Emanuel.
The trio combined for the winning goal, as Brenner and Adson stretched the defense before Paulo Henrique delivered the assist for Thiago Mendes, whose powerful finish beat goalkeeper Éder Chaux.
Medellín threatened but couldn't find an equalizer
The Colombian side created several chances through Kevin Mantilla and Jeison Medina, but failed to break through Vasco's defense.
Medina came closest when he headed the ball inside the box, only to see his effort drift wide of the post.
Shortly before the winning goal, Vasco appealed for a penalty after a possible handball inside the area. The play was reviewed by VAR, but no penalty was awarded.
Independiente Medellín pushed forward in the closing minutes but could not avoid elimination, while Vasco celebrated its first victory in six matches across all competitions.
